Percutaneous Tibial Nerve Stimulation
(PTNS)
Overactive bladder symptoms of urinary urgency, frequency, nocturia and urge incontinence are often treated with numerous approaches. These can include expensive life-long medications, behavior modification and biofeedback. Surgically implanted neurostimulators can also be placed near the spine, bringing with them possible surgical side effects and complications.
Kenosha Urology Clinic is now offering a new technique to our patients called Percutaneous Tibial Nerve Stimulation (PTNS). Using the tibial nerve near the ankle, PTNS indirectly stimulates the sacral nerve plexus, which regulates bladder function. The procedure, carried out in the office, involves placing a very thin needle near the tibial nerve which is then connected to a battery powered stimulator for a period of 30 minutes. The stimulation causes impulses to travel from the tibial nerve to the sacral nerve plexus. Approximately 70% of patients achieve significant improvement in bladder control. We have been impressed with our results to this point.
We feel this new office-based treatment is an excellent alternative for patients who are reluctant to use anticholinergic drugs, either because of possible significant side effects or associated high costs. In addition, patients with overactive bladder may not be good candidates for a sacral nerve surgical implant or will choose to not have this invasive procedure with its significant complication rate.
We and our patients have been impressed not only with the simplicity of PTNS but also its efficacy.
